By giving your consent through the form below you freely and unambiguously:

  1. Confirm that you have read this consent form, understand the explanation given on the data processing and all questions regarding this processing have been clearly answered by WPL, i2connect or Wageningen University & Research;
  2. Confirm that you agree to be included in the database;
  3. Understand that all information collected during the field peer reviews will be recorded and/or documented. In the meeting you will be asked to share data about your case via an interview, a field peer review visit and additional feedback and reflection discussions. All information will be held in the strictest of confidence. Results from the research will be reported as aggregate data to the REA and will be used to influence the project development and the i2connect AS database. However, it is possible specific cases might be used without names and details, but due to the nature of the case might be relatable. At the end of the project period, data will be retained by the consortium members and will not be sold to a third party but could be shared for other project tasks and activities.
  4. Data can be fully anonymized and reused for other researches, such as a follow up on this project. As data is anonymized the data is no longer considered as personal data and therefor the GDPR is no longer applicable
  5. Understand that any personal data collected in the field review will be recorded and stored in accordance with the General Data Protection Regulation (GDPR). A copy of the i2connect data privacy policy should be made available by the organizers.
  6. Are aware you have a limited right to revoke your personal data due to the nature of collecting and processing data in terms of research.
